Job Description:
Practices the profession of pharmacy with the utmost code of ethics demonstrating the organization's mission and values.
Responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in the ICU.
Use advanced knowledge, skills, and professional judgment in all aspects of their work.
Serve as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner.
Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.
Document all clinical activities and interventions accurately and completely.
Complete notification reports regarding medication errors and adverse drug reactions.
Education:
BS or Masters in Pharmacy.
Graduate of a college of Pharmacy accredited by the American Council on Pharmacy Education.
License/ Certifications:
Active pharmacist license in good standing with the appropriate State Board of Pharmacy
Experience:
Minimum of 3 yrs experience and previous exp or residency in Infectious disease or Critical care.
